A 46 year old gentleman had involved in a motor vehicle accident and sustained intracranial bleeding with multiple facial bone and cervical spine fracture. Tracheostomy was done due to poor GCS improvement. Post tracheostomy chest x-ray revealed aspirated tooth in the right lower bronchus. Removal using flexible bronchoscopy had failed. Due to hard collar and cervical spine fracture, rigid bronchoscope was introduced through pre-exesting tracheostomy stoma and foreign body was successfully removed through this approach.
